HEMLIBRA® im Einsatz bei Säuglingen und PUPs
HEMLIBRA® bei Säuglingen, PUPs und weiteren Patientenpopulationen
Säuglinge (von der Geburt bis zu einem Alter von 12 Monaten) mit HA erhalten häufig keine Prophylaxe und sind daher nicht ausreichend vor Blutungen geschützt.2–4
Emicizumab ist für die Prophylaxe bei allen Personen mit schwerer HA zugelassen, einschließlich Säuglingen, unabhängig von Alter und FVIII-Hemmkörperstatus.1
MASAC-Empfehlung: Bei Säuglingen sollte aufgrund des erhöhten Risikos für intrakraniale Blutungen nach der Geburt und vor Beginn der FVIII-Prophylaxe eine Prophylaxe mit Emicizumab (HEMLIBRA®) in Betracht gezogen werden.5

HAVEN 7: Klinische Studie mit Säuglingen im Alter von <= 12 Monaten, einschließlich PUPs
Die Studie HAVEN 7 (NCT04431726) untersuchte die Wirksamkeit, Sicherheit, Pharmakokinetik und Pharmakodynamik von HEMLIBRA® in dieser vulnerablen Patientenpopulation.6
Studienziele
- Wirksamkeit: Blutungsbezogene Endpunkte (einschließlich Rate behandelter Blutungen, Rate aller Blutungen, Rate behandelter spontaner Blutungen und Rate behandelter Gelenkblutungen) und Gelenkgesundheit6
- Sicherheit: Inzidenz von UE (und Schweregrad gemäß WHO-Skala zur Bewertung der Toxizität), thromboembolischen Ereignissen oder thrombotischen Mikroangiopathienn, UE, die zum Absetzen von Emicizumab führen, Reaktionen an der Injektionsstelle, schwerer Überempfindlichkeit, Anaphylaxie und anaphylaktoiden Ereignissen6
- Pharmakokinetik und Pharmakodynamik: Plasmakonzentration, Biomarker (einschließlich aPTT, Tg-Peakhöhe, FVIII-ähnliche Aktivität) und Konzentrationen der HEMLIBRA®-Targets (FIX- und FX-Antigenspiegel)6
- Immunogenität: Inzidenz und Signifikanz von Anti-Emicizumab-Antikörpern sowie De-novo-Entwicklung von FVIII-Hemmkörpern6

Gute Verträglichkeit von HEMLIBRA®
Die Sicherheit und Verträglichkeit von HEMLIBRA® bei Säuglingen war in der Studie HAVEN 7 (N = 55), der pädiatrischen Subanalyse von TSUBASA (N = 46) und der 3-Jahres-Zwischenanalyse von AOZORA (N = 30) konsistent6,13,23:
* Patient:innen mit ICH waren von der Studie ausgeschlossen. Die Studie war nicht darauf ausgelegt, Nachweise für den Schutz vor ICH durch die Prophylaxe mit HEMLIBRA® zu erbringen.6
